Post-offsite survey questions (company retreat feedback template)
Retreat feedback questions that tell you whether the offsite worked: outcomes, sessions, connection and logistics, with rating scales and a short version.
A post-offsite survey asks attendees, within a few days of the retreat, whether the offsite achieved its goals, which sessions were worth the time, whether they feel more connected to colleagues, and how the travel, venue and food worked. Keep it to five to ten minutes, mix rating scales with two or three open questions, and ask the same core questions every year so you can compare. Below is a free post-retreat survey template you can import into Google Forms, Typeform or your HR tool.

3. Sending and reading the survey
Sending
- Send within two days of returning, while it is fresh
- Say how long it takes and that answers are anonymous, if they are
- Send one reminder, then close the survey after a week
Reading the results
- Separate logistics scores from content scores: they have different fixes
- Read every open answer; quote themes, not individuals
- Compare core questions with last year and with the pre-offsite survey
- Share three things you learned and what will change next time
Opens in Google Sheets, Excel, Notion or Google Docs. Paste the Markdown into Notion or a doc, or import the CSV into a sheet.

How to use this post-offsite survey
- Before the offsite, decide what success looks like and make sure the core questions measure it.
- Pair it with the pre-offsite survey so connection and clarity scores have a baseline.
- Import the CSV into your survey tool and trim to 12 to 18 questions, or use the five-question version for small teams.
- Make it anonymous unless you need names for follow-up, and say so at the top.
- Send within two days of the return, with one reminder.
- Share a short summary with attendees and leadership, including what you will change. People answer future surveys when they see past ones acted on.
Example: reading results from a Lisbon retreat
An illustrative summary from a 30-person retreat in Lisbon. Numbers are made up to show how to read the survey, not real results.
| Signal | What the survey showed | What we change next time |
|---|---|---|
| Value | Most people rated the offsite 4 or 5 | Keep the format |
| Clarity | Many were unsure who owned two of the decisions | Read back the decision log with owners before leaving |
| Connection | Cross-team connection rose versus the pre-offsite survey | Keep mixed tables at dinner |
| Logistics | Arrival day felt rushed after overnight flights | Start sessions after lunch on day 1 |
| Open answers | Several asked for more unstructured time | Add a free afternoon |
Common post-offsite survey mistakes
- Waiting two weeks. Memory fades and response rates drop. Send it within two days.
- Asking only about logistics. Hotel scores don't tell you whether the offsite achieved anything.
- Changing the questions every year. Keep the core set fixed so you can compare.
- Leading questions. "How amazing was the dinner?" produces flattering, useless data.
- Never sharing results. If nothing visibly changes, people stop answering.

FAQ
What questions should I ask after a company retreat?
Ask about overall value, clarity on decisions, personal next steps, connection with colleagues, the most and least valuable sessions, travel and venue, and one open question about what would make the next offsite more useful.
When should I send a post-retreat survey?
Within two days of the group returning, with one reminder a few days later. Close it after about a week.
How long should an offsite feedback survey be?
Five to ten minutes: 12 to 18 questions for a full survey, or five questions for a small team. Long surveys get rushed answers.
Should a post-offsite survey be anonymous?
Usually yes, because people are more candid about sessions and leaders. If you need names to follow up on logistics issues, add an optional contact field.
How do I measure if an offsite was successful?
Compare the core survey answers with the goals you set before the trip, track connection and clarity against a pre-offsite baseline, and check at 30 days whether the decisions made at the offsite are moving.
