Offsite action plan template: debrief, decisions and follow-up
What to do in the week after an offsite: a decision log turned into owners and dates, a short debrief, the recap email to the company and 30-, 60- and 90-day check-ins.
An offsite action plan turns what was decided at a team offsite or leadership retreat into owners, dates and check-ins, so decisions survive the first busy Monday back. It has three parts: a decision log with owners and first milestones, a short debrief on the offsite itself, and a follow-up rhythm at 30, 60 and 90 days. Below is a free offsite action plan template, a debrief agenda and a recap email you can send to the wider company.
Offsite action plan template
Fill it in during the last session of the offsite, not afterwards. The owner of each line should write their own first milestone.

| 30-40 min | What do we change next time? | Survey open answers |

How to use this offsite action plan
- Reserve the last 30 to 45 minutes of the offsite to fill in the decisions table together, with each owner writing their first milestone.
- Read the table back aloud before anyone leaves. Disagreement is cheaper in the room than in a follow-up thread.
- Send the recap email to the wider company within a week, so people hear decisions from leadership, not rumors.
- Run the 45-minute debrief with the post-offsite survey results in hand.
- Put the 30, 60 and 90-day check-ins in calendars before the debrief ends.
- At day 90, start the next offsite plan with the debrief notes, while the lessons are fresh.
Example: an action plan after a Seattle leadership offsite
An illustrative plan for an eight-person leadership team after a 2-day offsite near Seattle, a city Offsiteio has planned offsites in.
| Decision | Owner | First milestone | Day 30 status |
|---|---|---|---|
| Focus new sales on mid-market accounts | Head of Sales | Updated target account list | On track |
| Launch a self-serve tier | Head of Product | Pricing proposal to leadership | At risk: needs finance input |
| Hire a head of customer success | CEO | Job description approved | Done |
| Stop custom builds for single customers | CTO | Policy shared with sales | On track |
The self-serve tier was the line at risk at day 30, so it was the first item at the 60-day review, with finance in the room. The point of the plan is to surface that kind of blocker early.
Common offsite follow-up mistakes
- Writing the action plan after the offsite. Owners who didn't write their own milestones don't own them.
- Silence to the wider company. If leaders don't share decisions, people fill the gap with guesses.
- No dates on check-ins. "We'll review progress" without a calendar invite never happens.
- Too many actions. Thirty actions means none get done. Keep it to the decisions that matter.
- No debrief on the offsite itself. Without one, next year repeats this year's logistics problems.

FAQ
What is an offsite debrief?
A short meeting within a week of the offsite where the sponsor and organizer review whether the offsite met its goals, what attendees said in the survey, what worked in format and logistics, and what to change next time.
How do you follow up after a leadership offsite?
Finish the offsite with a decision log that has owners and first milestones, send a recap to the company within a week, and hold check-ins at 30, 60 and 90 days.
What should an offsite recap email include?
Thanks to those involved, the main decisions with owners and first steps, what you will stop doing, what it means for the reader and when progress will be shared next.
Who owns the offsite action plan?
Each decision has one named owner. The sponsor, usually the CEO or Chief of Staff, owns the plan as a whole and runs the check-ins.
How soon after an offsite should the action plan be shared?
Within a week. After that, priorities from before the offsite take over again and decisions start to blur.
